
LIVING ASSET MAP (LAMP)
Developed by SingHealth Community Hospitals and Singapore Land Authority, the Living Asset Map (LAMP) is an interactive and dynamic platform that maps community resources in addition to Active Ageing Centres and other officially listed resources. Through controlled crowdsourcing and geospatial mapping powered by OneMap, the platform goes further to map an up-to-date database of local assets that has included ground-up activities such as crochet and knitting classes, chess, senior pickleball and handicraft nights. Social prescribing practitioners from hospitals, Active Ageing Centres and other interest groups can easily access these resources to provide community-based interventions for their patients and clients.
The LAMP strengthens the practice of social prescribing through empowering practitioners to connect patients to a wide range of non-medical community support that addresses social determinants of health in an efficient, collaborative and sustainable manner. The LAMP has also been recognised at the ESRI Singapore GeoInnovation Award* in 2024, highlighting how geospatial technology can enhance integrated, community-centred healthcare in Singapore. It enables social prescribing practitioners and healthcare teams to use and contribute to the map effectively, ensuring timely and accurate referrals to community programmes.
